The Application Process
There are three ways that you may start the application process:
1. Online - using the form directly below this paragraph.
When you click the submit button the information you have typed will automatically be sent to the staff at Second Stage Housing. After you click submit you will see a page that tells you your form has been submitted.
2. Download the PDF below, print it, complete the information by hand then mail or fax (519-637-2213) it to the address or number shown on the form.
3. Download the Word file below, complete the information and mail or email the document to: thsp.stessh@rogers.com
